Belper PCN is a Primary Care Network established in 2019, bringing together four GP practices in Derbyshire: Appletree Medical Practice, Arthur Medical Centre, Riversdale Surgery, and Whitemoor Medical Centre. The network serves communities across Belper, Duffield, and Horsley Woodhouse. The PCN delivers integrated care through a multidisciplinary team including clinical pharmacists, mental health practitioners, social prescribers, nurse prescribers, healthcare assistants, and emergency care practitioners. Services include a home visiting Team Up service for housebound patients, enhanced access appointments, and clinical pharmacy support.
