About Chester Central Primary Care Network

Chester Central PCN brings together six GP practices serving over 41,000 patients across Chester. The network coordinates care delivery, enabling patients to access dietitians, wellbeing coaches, mental health counsellors, physiotherapists and clinical pharmacists alongside traditional GP services. This collaborative approach helps patients with complex conditions receive coordinated support from multiple specialists. The network eases pressure on individual GP practices while improving patient access to healthcare. By working at scale, the six practices offer services that would be difficult to provide individually, support better outcomes for patients with long term conditions, and move care towards a more preventative model.
