Coventry Navigation 1 Primary Care Network (PCN) brings 11 Coventry GP practices together to provide coordinated NHS primary care. Its shared team includes care coordinators, clinical pharmacists, first-contact physiotherapists, paramedic practitioners, physician associates and social prescribers, supporting patients with clinical, mental-health, social and practical needs. By sharing staff, expertise and technology, the network aims to improve health outcomes and patient experience while keeping care personalised and closer to home. Its model helps people with complex or long-term conditions navigate health and social care, expands access through Enhanced Access appointments, and connects communities with local non-clinical support.
