Derbyshire Community Health Services NHS Foundation Trust provides specialist community health services to over 1.1 million people across Derbyshire and Derby. The Trust employs approximately 4,400 staff delivering care in 11 community hospitals, over 30 health centres, GP practices, schools, care homes, and patients' own homes. Services include community nursing, urgent treatment centres, therapies, mental health support, and specialist clinical care. Rated Outstanding by the CQC in 2019.
DCHS brings healthcare closer to where people live, reducing hospital visits. The Trust runs urgent treatment centres seven days a week, provides personalised care plans, and increasingly offers virtual consultations alongside face to face services.
