
Durham Dales Health Federation is a not-for-profit, clinically-led organisation established by 12 GP practices to deliver accessible, patient centred care across the Durham Dales. As an NHS provider, DDHF employs 100 healthcare professionals offering same day urgent care, mental health support, vulnerable adult care, social prescribing, pharmacy, and physiotherapy. These services reduce GP workload, prevent hospital admissions, and keep care local. DDHF partners with Primary Care Networks, NHS trusts, local authorities, and the voluntary sector. Rated Good by the Care Quality Commission, DDHF delivers neighbourhood healthcare and addresses health inequalities across County Durham through an integrated model ensuring continuity of care.
