Coventry GP Connect PCN is a Primary Care Network linking five Coventry GP practices: Parkhouse Surgery, Phoenix Family Care, Stoke Aldermoor Medical Centre, Windmill Surgery and Woodway Medical Centre. It gives patients access to a primary-care team, including care coordinators, pharmacists, physiotherapists, mental-health coaches, pharmacy technicians, physician associates and social prescribers. Working with community, mental-health, social-care, pharmacy, hospital and voluntary services, the network coordinates support as demand grows from longer lives, long-term conditions and mental-health needs. Its Enhanced Access service offers evening and Saturday appointments at City of Coventry Health Centre, extending access to primary care.
