Island City Network PCN is a primary care network serving patients across Island City Practice and Derby Road Group Practice. It delivers shared COVID-19 vaccination clinics, pharmacy and pharmacy technician services, physiotherapy and social prescribing through a multidisciplinary team that also includes mental-health facilitators and occupational therapists. By coordinating primary care with health and social care, the network provides more proactive, personalised and integrated support across its two practices. Its clinicians help with medicines, muscular and joint conditions, mental health, wellbeing and practical or social needs, connecting people with community services and specialist care when appropriate.
