Primary Care Sheffield is a not for profit organisation set up by general practices to strengthen primary care in Sheffield. It runs GP practices, provides integrated home care, and delivers at scale services including extended access appointments, specialist clinics, and health improvement programmes. With 79 GP practices as shareholders covering over half a million patients, it provides back office support and tackles health inequalities.
Primary Care Sheffield ensures patients receive coordinated care closer to home while giving GP practices operational support to remain sustainable. The organisation pioneers new service models and has achieved an Outstanding CQC rating for improving health outcomes and reducing barriers to care.
