Shrewsbury Primary Care Network is 11 GP practices working together for over 104,000 people across Shrewsbury. Its shared teams provide joined-up support including first-contact physiotherapy, paramedic home visits, clinical pharmacy, advanced nursing, social prescribing, mental-health care and extended evening and weekend access. By sharing staff, resources and expertise, the network makes care easier to access, helping patients see the right person at the right time and place. It works with NHS, council and community partners through an Integrated Neighbourhood Team to support people living with frailty to live and age well, while addressing health inequalities through cancer-screening support and education sessions.
