South Sefton PCN brings together 19 GP practices serving 161,000 patients across Bootle, Crosby, Maghull, Litherland and Seaforth. The network integrates GPs, pharmacists, social prescribers, care coordinators and advanced nurse practitioners to deliver proactive, personalised care. By working with community services, social care and voluntary organisations, patients access the right support at the right time.
Formed in 2022, the network strengthens general practice through shared resources and population health management. It tackles health inequalities by identifying at risk groups and providing targeted interventions from care home support to health checks. Services include acute home visiting, medicines management and mental health support.
