Its services include frailty care, urgent home visits, diabetes education, blood-pressure checks, medication reviews, social prescribing and wellbeing coaching, including support for housebound patients and care-home residents. Through collaborative, person-centred work as a Primary Care Network and Integrated Neighbourhood Team, it aims to improve health outcomes, tackle inequalities and make care more equitable. Its teams connect treatment with practical and community support, helping people manage long-term conditions, reduce isolation, stay safe at home and avoid hospital visits.
