Wokingham North PCN, operating as Phoenix Health PCN, comprises three local GP surgeries: Parkside Family Surgery, Woodley Centre Surgery and Wargrave Surgery, with a combined registered population of around 38,000. The network was established in 2020 to enable practices to work more closely together. Phoenix Health PCN provides coordinated, community-based healthcare through a multidisciplinary team including GPs, nurses, paramedics, clinical pharmacists, social prescribers, health and wellbeing coaches and care coordinators. Services include enhanced access appointments, support for long-term conditions, proactive prevention programmes and closer integration with community services.
