Description
Job summary
This is an exciting time to join the Neighbourhood, Public and Mental Health System as it continues to grow and develop clinical services.
Following progression of the current Matron to Head of Nursing, we are looking to recruit an enthusiastic, dedicated, and passionate senior nurse to join our team as the Matron for the Integrated Children's Community Nursing Team (ICCNT).
The post holder will ensure co-operation and cohesion between the service's community nursing team, ensuring a seamless, positive experience of care for children and their families, which transcends community and hospital-based care. Involvement in the development of Integrated Community pathways to promote hospital avoidance, admission and early discharge is key to this role.
The post holder will provide strong leadership across the system enabling efficient coordination of safe and skilled multi professional involvement ensuring that the trust has robust, sustainable, effective governance systems and processes which comply with best practice.
The post holder will work closely with the Deputy Director of Nursing and AHPs to develop new and creative pathways to bring care closer to home in line with the Trust's 2030 Vision. Furthermore, the Matron will promote a culture of continuous quality improvement within all areas of the system and organisation to ensure best practice is achieved in the delivery of high-quality patient care for children, young people, and their families.
Main duties of the job
The post holder will provide clinical leadership and be accountable for the management, delivery, and development of the Integrated Children's Community Nursing Team, which provides nursing care in the community to children with acute/short term illness, those with complex health needs and disabilities, or with long term conditions, as well as those with life limiting conditions who may require end of life care.
Develop the service to, as a minimum, meet pertinent professional and national standards, including those concerned with workforce development and planning.
Lead on the development of the virtual ward and linked workstreams. This involves liaison with Innovation and external stakeholders.
Assume overall responsibility for the quality of the service, as experienced by children, their families and by colleagues.
