An exciting and rewarding development opportunity at Alnwick Medical Group!

We are seeking to employ a Frailty Practitioner. This is work alongside our current Frailty practitioner to help and support our local care homes.

Prescriber not essential.

The role would involve home visits within the community and residential homes. We aim to deliver continuity to patients and build rapport with our local residential homes to deliver excellent patient care.

We currently have 5 residential homes within the practice area.

We aim deliver excellent care to our high risk frailty patients to try and reduce and avoid hospital admission where necessary, working alongside our local community matron, frailty team within secondary care, GP's and also local geriatrician.

Main duties of the job

• Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
• Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
• Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans.
• Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
• Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
• Develop Emergency HealthCare Plans, discuss DNAR's - working with patients and families.

Job responsibilities
The Frailty Practitioner will provide clinical care to patients within their home environment.

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Develops policies and procedures which relevant for local and collaborative working and clinical improvement.

Carries our any other reasonable duties within the overall function, commensurate with the grading and level of responsibility of the post.

Ensures self-compliance with data protection laws and confidentiality agreements and that of the Nursing Team

Follows confidentiality policy, computer use policy, information governance policy, social media policy and any other policy relating to confidentiality within role.

• Working with the practice management monitor and develop the performance of the GPN team in accordance with local policies
• Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance

Follows Alnwick Medical Group policies and procedures relating to health and safety

Understands and uses the Safeguard, Incident & Risk Management System (SIRMS) when necessary

Effectively manages any health and safety issues raised.

Effectively communicates with patients and colleagues

Comfortably and confidently communicates verbally, as well as written methods including email

Qualifications
Essential

• Registered first level nurse MSc or equivalent
• Relevant nursing/health degree

Desirable

• Clinical Skills
• Previous experience within frailty/community.
• Knowledge of local and national public health issues.

Experience
Essential

• Experience of working with vulnerable/frail patients.
• Experience working within nursing team.

Desirable

• Experience working within community/primary care.
