Job summary
CARDIAC SPECIALIST NURSE (BAND 6)
24 hours a week - Permanent Appointment

A position has arisen to join the Cardiac Risk Management /Secondary Prevention & Rehabilitation Service team at Ashford & St Peter's Hospitals NHS Foundation Trust

Applicants are invited from experienced cardiac nurses to specialise in this rewarding area of cardiac nursing.

The post holder will manage their own case load and will empower patients to participate in the management and secondary prevention of their condition aiming at improving longevity and quality of life.

We are currently advertising for up to 24hours a week.

Main duties of the job
  • The purpose of this post is to contribute to an expert specialist service that positively impacts on the experience of patients & carers by providing high standards of Risk Management / Secondary Prevention & Rehab Services to ensure long term management of CHD & other cardiac conditions as appropriate across the trust
  • To be responsible for the assessment of patient care needs, the development, implementation and evaluation of programmes of care.

Job responsibilities
Clinical or technical
  • To provide specialist advice & support to multi-professional teams to improve the way in which services are provided
  • To assess patients specialist health & wellbeing needs at all stages of their treatment
  • In liaison with the Advanced Specialist Practitioner to plan, deliver & evaluate programmes to address patients specialist & changing health critical needs of all stages of their treatment.
  • Provide specialist advice to support ward staff when patients are referred to Cardiac Rehab, ensuring all medical & nursing needs are understood & carried out in accordance with present standards & protocols.
  • In liaison with the Advanced Specialist Practitioner to be responsible for managing a clinical case load
  • To be a nurse prescriber in the area of specialism as required and as determined by the Advance Specialist Practitioner, Head of Nursing & Non-medical prescribing & Patient group directions group.
  • Establish an effective communication system with patients, their carers, and all staff working in the multidisciplinary team.

Qualifications
Essential
  • Nursing Degree
  • NMC Register
  • Years experience in cardiac nursing

Desirable
  • Experience in cardiac rehab
  • Can manage own caseload
