Come and join our friendly team as a Senior Mental Health Practitioner for the Crisis Resolution Home Treatment Team East in the vibrant town of Slough. Our main mission? Your role will involve conducting thorough assessments, delivering effective treatments, and offering brief, evidence-based follow-up services to patients referred from community mental health teams, the psych liaison service, or inpatient wards.

While you'll be based in heart of Slough, get ready for some exciting variety as you might occasionally travel to other locations like Windsor, Maidenhead, or Bracknell to meet the diverse needs of our services.

Main duties of the job
Responsibilities include carrying out assessments, developing, implementing, and evaluating individualised care plans, along with directing patients to other services when necessary. Collaboration with service users and the broader multidisciplinary team is integral to the development of each personalised plan.

Operating within a multidisciplinary team, you'll oversee care delivery, manage risks, and evaluate care outcomes, prioritising the recovery of each service user.

You should be adept at taking calculated positive risks, aligning with the Trust Policy on Risk Assessment and Management.

Job responsibilities
The "must haves" for this role:

• Registered Mental Health Nurse or social worker.
• Evidence of a sound understanding of the acute care pathway.
• Experience of assessment and case formulation, evidence of skills utilised to manage risk.

• NMC Registration
• Specialist knowledge acquired through post graduate short courses, training and equivalent experience, in a relevant field.

Previous Experience

• Demonstrable experience of working autonomously to deliver high quality and evidence-based nursing care.
• Experience in providing supervision to junior colleagues and students in the completion of routine nursing and clinical skills.

Knowledge, skills and abilities
Essential

• Demonstrable understanding of an acute mental health environment and relevant legislation such as the Mental Health Act, Mental Capacity Act and DOLs.
• Proficient use of digital tools and applications including Word, Outlook and patient management systems.

• Ability to travel between multiple Trust sites and non-Trust locations, including patient homes, some of which may not be easily accessible via public transport.
