We are modernising the way that community mental health care is delivered in Sandwell, Dudley, Walsall and Wolverhampton. This truly transformational role will sit within our referral support service, linking both community mental health teams and primary care mental health teams.

Main duties of the job
The successful candidate will play a pivotal role in community outreach, facilitating education and engagement initiatives for patients accessing services and finding the most suitable referral pathways.

The Clinical Care Navigation Manager will work closely to support seamless processing of BCHFT referrals where the referral pathway is not clear. They will be the point of contact for professionals, ensuring accurate pathways for referrals into BCHFT Adult Services, timely patient appointments, and appropriate levels of care will be a core responsibility.

Offering expert guidance to professionals on progressing referrals, the role involves collaborating extensively with clinical colleagues across all All-Aged adult BCHFT mental health services. Every referral, be it for accessing a BCHFT service, redirection to an alternative service beyond our scope, or providing valuable advice for support, will receive dedicated attention.

Job responsibilities
The role involves close collaboration with partners in the Voluntary Sector and Local Authorities across the Black Country to improve pathways for All-Aged Adults who might not meet the threshold for BCHFT services but could benefit from alternative interventions and support.

The successful candidate will possess a clinical background, tasked with coordinating and steering the implementation of BCHFT Trust-wide and Local Referral Support HUB SOP policies. This involves extensive consultation and collaboration with Responsible Clinicians, their teams, GPs, broader PCNs, Care Coordinators, Primary Care Managers, Operational Managers, VCSE Managers, and senior management.

To actively manage referral management capacity within BCHFT ensuring timely triage and assessment and clinically appropriate care plan development.

To lead in the process of managing patient flow, coordinating their journey through each referral pathway ensuring involvement of Medical, nursing and administrative staff.

To work closely with clinicians and team managers to ensure that patients only remain on the waiting list for assessment for the necessary period required to assess and confirm care plans for their mental needs keeping waiting times to a minimum.

To utilise advanced communication skills, verbal, non-verbal and written when dealing with highly charged and complex situations. To communicate sensitively and empathetically and diffuse situations of conflict when tensions arise around clinical priorities.

To attend and contribute to relevant meetings within the Directorate, ensuring positive engagement of all members of staff with the problems associated with capacity management.

To lead on the development of appropriate audits in relation to referral management and the development of action plans in light of findings.

In certain circumstances, the post holder will be expected to complete and document clinical triage; assessments and associated paperwork to support the referral support & management process.

Have the authority to work directly with Responsible Clinicians and GPs, senior team management / Clinical Leads and Senior Managers and will be accountable for ensuring that the decision making process is effective and that remedial action where necessary is proposed to the senior team.

The post holder will advise on the optimal use and cost effective management of triage & Assessment Services/resources for patients in accordance with BCHFT financial plans and business priorities.

The role will cover all Mental Health routine referrals into the Trust and will work in accordance with the referral management guidance and E-referral System Criteria within the BCHFT Referral Support HUB Policy.

Will provide training on referral management initiatives or changes to pathways and practice to Responsible Clinicians, GPs and their team, Care Coordinators, Team Managers, and senior management.

Person Specification
Education, Knowledge, skills
Essential

• Degree level qualification and or relevant experience
• Registered nurse - mental health (RMN) current with the NMC
• Working within a target driven environment and within strict time limits
• Risk management experience
• Safeguarding and Vulnerable Adults training
• Experience working with people with mental health needs
• Extensive knowledge and or experience of Mental Health Assessments, Primary Care and related interventions
• Experience of accessing and updating information in electronic and paper clinical system
• Ability to build and maintain effective working relationships with team, other services, partner agencies and stakeholders
• Ability to organise and respond efficiently to complex information

Desirable

• Involvement in research and/or clinical evaluation and audit
• Experience working in the community and with commissioners
