Job summary
Provide clinical expertise regarding admission and discharge criteria and ensure bed management policies and procedures are adhered to.

To be a part of the multi-disciplinary team, influencing responsive and timely inpatient admissions where length of stay is proportionate to need.

The process of enhancing Patient flow is accomplished by an improved admission process, comprehensive discharge planning and coordination with relevant clinical and non-clinical services.

The post holder will be expected to utilise their clinical and risk assessment skills to ensure the best outcomes for patients as part of the admission and discharge process

Main duties of the job
The Discharge Coordinator will be expected to contribute a clinical opinion, based on their clinical skills and knowledge, as part of the multi-disciplinary team to support a timely and effective step down of patients from an in-patient setting, for both patients admitted within and outside of Trust.

The post holder will be expected to utilise their clinical and risk assessment skills to ensure the best outcomes for patients as part of the admission and discharge process.

Ultimately the Discharge Coordinator will

• Attend MDT ward reviews
• Identify any blockers to a patients discharge in a timely manner in order to identify clear exit pathway
• Support reduction in Length of Stay on the ward
• Attend Clinically Ready for Discharge, MADE meetings, locality meetings and other meetings pertaining to flow
• Be a point of escalation for wards
• Identify patient needs and to liaise with internal and external agencies to support safe discharge
• Be responsive to Trust OPEL score

We are responsible for the accepting bed referrals, allocating of beds and supporting safe discharges. The Team out of hours are supported by the UCR team making a seamless Team approach
