Job summaryWe welcome applications to join us at the Community Mental Health Assessment Team.
The Post Holder will be responsible for the triage/assessment of routine referrals to the assessment team, signposting to other agencies when appropriate and to the wider CMHT.
They will hold a short term caseload and be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care.
The post holder will provide high standards of skilled mental health care to individuals and their families. The role will include active participation in the service planning and development of the nursing and multi-disciplinary teams.
The post holder will provide clinical supervision and support to junior members of the team and actively promoting the values and policies of the service and the Trust.
The Community Mental Health Practitioner will be an active member of the multi disciplinary team and will work in close co-operation with secondary and primary care based professionals, other community agencies such as Housing, Probation, Police, Children Schools & Families and other Voluntary and Third Sector Agencies.
Community Mental Health services are fully integrated teams that are operated jointly with Bradford Metropolitan District Council and all staff within the teams will be expected to deliver health and social care outcomes for service users.

- To triage and assess routine referrals into the CMHT
- To input data in the clinical information system: e.g. including risk assessments, care plans, CPA, crisis and contingency plans
- To work as a part of duty/day co-ordinator service within the CMH AT
- To supervise and mentor student nurses
- To demonstrate effective networking skills and ensure a holistic approach to meet service user needs.
- To work to ensure the safety and wellbeing of service users and their families
- To work in partnership with service users and carers to achieve goals and to support recovery and reintegration within communities
- To provide appropriate feedback and liaison with MDT on all aspects of service user care.
- To ensure that agreed standards of nursing care within the team are achieved.
- To actively participate in the flexible formats of team supervision and support.
- To initiate, develop and assist in the trial of new ideas for the improvement of services to service users in conjunction with the multidisciplinary team.
