Are you looking for a role where no 2 days are the same?

We are looking for motivated and compassionate Band 5 Community Nurses to join our dynamic locality teams. This is an exciting opportunity to deliver high quality person centered nursing care to people in their own homes, supporting them to remain independent, safe and well within their community.

This role is ideal for Qualified and newly qualified nurses, those seeking their first community post, or nurses wanting to broaden their skills and progress with locality teams, within a well structured career pathway.

You will be offered a structured preceptorship and as a community nurse working within the planned care service , you will be providing high quality care for people in their own homes, ranging from long term conditions, to complex and palliative care. You'll be working on a rotational basis as part of a seven-day service, with the support of agile technology to help you deliver timely, high quality care, with support from specialists across the Trust.

Shift patterns are between the hours of 8am and 9pm, 7 days per week and We have part time vacancies within the North locality teams.

Main duties of the job
There are lots of development opportunities available to you as a community staff nurse to enable to progress your career and offering promotion to senior nurse or further study to become a qualified specialist practitioner or advanced practitioner, Along with other modes of study to enhance your skills in tissue viability or palliative care as a few examples.

Applicants must be able to regularly travel extensively across the district to meet the needs and demands of the service, therefore a full UK driving license and access to a car for work is essential

Main Duties:
To work as part of the district nursing team to ensure high quality health care is delivered to patients in their own home and/ or clinic setting. To work in collaboration with the wider Primary Health Care team, whilst participating in the assessment of health needs and delivery of care to the practice population. The service is delivered over a 7 day rota and postholders are expected to work flexibly in line with agreed operating hours to cover a 24 hour service

Essential
  • Understanding and/or experience of interagency working and adult protection.

Qualifications

  • RGN
  • NMC registration active

Desirable
  • Teaching / Assessing in clinical practice
  • V100 prescribing for nursing or equivalent

Training

  • Willingness to undergo education / training for both practice and service need

  • Demonstrates ongoing professional development
  • Must be able to travel between multiple sites to undertake numerous visits,
