We're looking for an enthusiastic and motivated band 6 practitioner to join our developing Community Rehabilitation Team working across the Bradford and Craven district.

At BDCFT, we're committed to reducing health inequalities and improving outcomes for people living with Severe Mental Illness (SMI) through high-quality, recovery-focused rehabilitation services.

In this role you will be pro-active in reviewing and engaging those with complex psychosis and SMI in their rehabilitation journey .

You will work closely and collaboratively with the Community Mental Health Teams and partner organisations across the district, you'll play a key role in building strong relationships, coordinating care, and ensuring people receive the right support at the right time.

Main duties of the job

• To manage a clinical caseload as required to meet the rehabilitation needs of service users.
• To support patient flow and development of rehabilitation pathways as defined by the leadership team
• To implement evidence-based specialist rehabilitation knowledge and skills
• To be a proactive team member in respect of communication, team administration, quality of service and service development.

Person Specification
Training and Qualifications
Essential

• Degree or equivalent pre-registration award in Nursing, AHP, Social Work, Psychotherapy
• Short specialist course/s with relevance to specialist clinical area
• State registration with NMC, HCPC, BACP
• Full current UK Driving License
• Professional practice educator training

Desirable

• Professional Practice education accreditation
• Leading an Empowered Organization (LEO) training or equivalent
• Level 2 Chartered Management Institute training or equivalent

Experience
Essential

• Working as an Mental Health Practitioner assessing the needs of service users with severe and enduring mental health problems.
• Mental health rehabilitation
• Clear supervisory experience.
• Professional practice education experience

Desirable

• Post registration experience of working with a range of mental health problems i.e. psychosis and personality disorders
• Experience working with individuals having neurodevelopmental disorders
• Experience of working within an inpatient setting.
• Conducting staff reviews

Health and Well-Being skills
Essential

• Clear understanding or working knowledge of NICE guidance relative to clinical area & quality indicators.
• Experience of using a variety of specialized assessments and models
• Experience of using outcome measures appropriate to the specialist clinical area.
• Able to relate to and empower people to realize and maintain their potential in relation to health and wellbeing.
• Ability to assess function, ability, risk and interdependencies with psychosocial needs contributing to overall assessment of health and well being needs.
• Ability to create, monitor and review treatment plans, taking into account the complex, serious and changing needs of individuals.
• Effectively utilize a wide range of specialist therapeutic skills to provide treatment interventions, both through individual and group media.
