Description

Job summary
Join our expanding Home First Discharge Team and help make a real difference to patients' lives.

We have several exciting opportunities for motivated and compassionate Registered Nurses and Allied Health Professionals (AHPs) to join our dedicated team as Case Managers, supporting patients to leave hospital safely, timely and with the right care and support in place.

At the heart of the Home First approach is a simple belief: patients recover better at home, whenever it is safe and appropriate to do so. Prolonged hospital stays can impact a person's recovery, independence and wellbeing. As a Case Manager, you will play a vital role in ensuring patients receive the right support to return home or move to the most appropriate care setting as quickly and safely as possible.

Could this be the role for you?
  • Do you want to make a tangible difference to patients' discharge journeys?
  • Are you passionate about delivering person-centred care and improving patient outcomes?
  • Do you enjoy working collaboratively with multidisciplinary teams to solve complex challenges?
  • Are you looking for a role where you can influence and improve the patient and family experience?

If so, we would love to hear from you.

Main duties of the job
As a Case Manager, you will be a key member of our Transfer of Care Hub, coordinating and facilitating safe, effective discharge plans for patients across a range of specialties. You will work closely with ward teams, patients, families, community services and partner organisations to ensure seamless transitions from hospital to home or onward care settings.

This is a rewarding and varied role that requires excellent communication, clinical knowledge, problem-solving skills and the ability to work effectively across organisational boundaries.

What We Offer
You will join a forward-thinking, patient-centred service committed to continuous improvement and excellence in discharge planning. We pride ourselves on creating a supportive and collaborative working environment where staff are empowered to develop, innovate and share learning.

You will work alongside a diverse range of professionals, including:
  • Social Workers
  • Community Transfer of Care Hub (CToCH) colleagues
  • Community Therapists
  • Ward-based clinical teams
  • Community and voluntary sector partners

Together, we are committed to delivering the best possible outcomes for patients and their families.
