Job summaryWe have an exciting opportunity for someone with relevant experience to join the Dementia Delirium & Falls team as a Band 6 Dementia, Delirium & Falls Practitioner based at Weston General Hospital.
This is a dynamic clinical role ensuring high quality care for people with dementia, delirium & falls.
Main duties of the jobThe post holder will work as an important member of the Multidisciplinary Team within Bristol NHS Foundation Trust. The post holder will provide specialist nursing advice and support to in-patients and their family, throughout the care pathway. The post holder will utilise skills of clinical expertise, education and management to ensure a seamless service for patients, carers and staff. S/he will provide advice, support and information for people with dementia, delirium and/or at risk of falls.
They will work closely with the multidisciplinary team in hospital and the community to ensure a co-ordinated quality service and will be actively involved in education, audit and research.

Person SpecificationEducation & QualificationsEssential- Registered Nurse - first level registration, or relevant registered AHP/clinical qualification.
- Degree in nursing or a health-related subject, or willingness to undertake/work towards.

Desirable- Teaching and assessing qualification or equivalent relevant experience.
- Willingness to undertake Master's-level academic study

ExperienceEssential- Appropriate post-registration clinical experience relevant to the specialty

- Experience of undertaking clinical audit or research activity.
- Evidence of involvement in evidence-based practice, quality improvement or project work.
