Job summary
An exciting opportunity has arisen for a motivated nurse to join the Diabetes Team at Bristol NHS Foundation Trust. You will play a key role in supporting us to deliver high-quality inpatient and outpatient diabetes services across a diverse and dynamic clinical environment.

The role involves autonomous clinical practice under the supervision of the Senior Enhanced Practitioners, managing a defined caseload of patients with diabetes. You will contribute to assessment, care planning, education, quality improvement and service development.

We are looking for someone who demonstrates strong communication skills and a compassionate approach to care. A willingness to learn and contribute to a positive team culture is essential.

Main duties of the job
  • Manage a caseload of patients with diabetes across inpatient and outpatient settings, delivering high-quality, evidence-based care.
  • Undertake autonomous, independent clinical practice under the direct and indirect supervision of the Senior Enhanced Practitioner within the diabetes service.
  • Assess, plan, implement and evaluate care for patients with complex diabetes needs using enhanced clinical judgement and decision-making skills.
  • Provide specialist diabetes advice, education and support to patients, carers and healthcare professionals to promote effective self-management and positive outcomes.
  • Initiate and adjust diabetes interventions and treatment plans within scope of practice and Trust guidelines.
  • Work collaboratively within the multidisciplinary team to support safe discharge planning, coordinated care pathways and service improvement.
  • Act as a specialist diabetes resource and support the delivery of evidence-based practice across the organisation.
  • Contribute to education, audit, quality improvement and research activities to support the ongoing development of diabetes services.
  • Demonstrate professional accountability, compassionate care and effective communication at all times.

Job responsibilities
  • Manage a specialist caseload of patients with diabetes across inpatient and outpatient settings, providing high-quality, evidence-based care tailored to individual patient needs.
  • Undertake autonomous, independent clinical practice under the direct and indirect supervision of the Senior Enhanced Practitioner within the diabetes service, using enhanced clinical judgement and decision-making skills to manage complex situations safely and effectively.
  • Assess, plan, implement and evaluate care for patients with complex diabetes needs, supporting effective treatment plans and positive patient outcomes.
  • Provide specialist diabetes advice, education and emotional support to patients, families and carers, empowering individuals to self-manage their condition and make informed decisions about their care.
  • Initiate, review and modify diabetes interventions and treatment plans within scope of practice and Trust guidelines, including supporting the adjustment of insulin and oral diabetes medications.
  • Work collaboratively with the wider multi-disciplinary team to ensure coordinated care pathways, safe discharge planning and effective ongoing diabetes management.
  • Act as a clinical resource within the diabetes service, promoting evidence-based practice and supporting high standards of patient-centred care.
  • Maintain accurate, comprehensive and contemporaneous clinical records in accordance with Trust policies and professional standards.
  • Participate in diabetes education and training programmes for healthcare professionals and contribute to creating a positive learning environment within the service.
  • Contribute to clinical audit, quality improvement, service development and research activities to support the continuous improvement of diabetes care and patient experience.
  • Demonstrate professional accountability, compassionate leadership and effective communication skills, recognising limitations of practice and escalating concerns

Person Specification
Education and Qualifications
Essential
  • Working towards Post registration qualifications/ CPD in area of diabetes
  • Active Professional Registered Qualification with a Professional body e.g., NMC, HCPC

Essential and desirable criteria per person specification

  • Excellent organisation and leadership skills
  • Experience of management of the diabetes patient
  • Able to work within a team or independently

Desirable
  • Prepared to work towards the Non-medical prescriber (If appropriate for statutory profession).
  • Proven leadership experience
