Job summary
This is a unique and rewarding opportunity for a motivated Pharmacy Technician to join our team, in a split role between Pharmacy clinical services (anticoagulation and cardiac services) and the hospital discharge teams.

As part of the anticoagulant team, the successful candidate will play a vital role in the Bristol Royal Infirmary's Warfarin Clinic. The post holder will provide specialist clinical advice and support to both patients directly and to hospital colleagues, contributing to safe and effective anticoagulation therapy.

Within the discharge part of the role, the post holder will become a key contact for the Hospital Discharge Team and BRI Discharge Lounge, working to improve discharge pathways and patient flow from hospital to home or community care.

Main duties of the job

• To provide a specialist technical and clinical service to the pharmacy-run anticoagulant dosing service for patients within the catchment area.
• To provide specialist anticoagulant advice to healthcare professionals in primary and secondary care and to improve anticoagulation patient care at ward level.
• Review the daily pathway discharge profile and facilitate resolution of any pharmacy-related issues to prevent delays to patient discharge.
• Assist patients and carers in the Discharge Lounge with complex medication-related issues arising during the discharge process

Person Specification
Educations and Qualifications
Essential

• Registered Pharmacy Technician with the General Pharmaceutical Council
• Evidence of continuing professional development
• Medicines optimisation technician training or equivalent - completed or within a few months of completing
• Recognised qualification as accredited checking technician (ACT)

Desirable

• Leadership or supervisory qualification/courses e.g. NHS leadership academy/CPPE (A/I)

Experience
Essential

• Post-registration experience as a Pharmacy Technician in hospital pharmacy practice
• Experience of medicines optimisation and patient counselling
• Experience of multidisciplinary team working

Desirable

• Experience of supervising or supporting trainee staff
• Experience supporting patient discharge processes
• Experience working within an anticoagulation service
• Experience undertaking audit and quality improvement projects

Knowledge, Skills and Abilities

• Knowledge of the legal framework for the dispensing, prescribing and administration of medicines
• Experience using electronic prescribing, patient record systems and dispensing systems
• Ability to interpret clinical information and work within agreed protocols
