Job summary
A registered general nurse with specialist knowledge of patients with End Stage Renal Failure. To facilitate and co-ordinate care of patients who are approaching End of Life from their renal disease.

To provide specialist knowledge of symptom control, advice and support to patients and families who choose a conservative pathway for their kidney disease. Able to work independently and manage a caseload of patients within the community to minimize hospital admissions.

Liaise with both primary and secondary care Palliative care teams to ensure cohesive management of patients at End of Life in hospital and home setting.

Supervise junior staff. Provide teaching and support to all members of nursing team. To develop care pathways to deliver high-quality end of life care, increased training in symptom control in ESKD.

Essential

• Bachelor of science in Adult Nursing (Degree)
• Registered Nurse (Adult) with current NMC registration
• Evidence of post Registration education and relevant to nursing practice
• significant post registration experience within Renal and or similar
• Evidence of clinical governance and leadership
• Recent Renal Experience in dialysis and acute care setting

Desirable

• Recent study Degree/ MSc level or working towards
• A range acute experience in Nursing Care
• Experience of implementing clinical policies, procedures and guidelines
• Knowledge of patient safety, risk management and quality assurance processes
• Strong organisational and prioritisation skills. Ability to work under pressure and manage competing demands

Salary £39,959 to £48,117 a year per annum, pro rata for part time roles
