We have an exciting opportunity for two Community Staff Nurses to join our service in a new and innovative role working across our seven locality teams.

This role offers a different way of working, with a particular focus on supporting Healthcare Assistants to deliver high-quality care in patients' homes. Key areas of work include long-term condition reviews, phlebotomy, continence assessments and promoting patient independence to prevent hospital admission and support timely discharge.

Working closely with the wider multidisciplinary team, you will be expected to demonstrate initiative, embrace new ways of working and contribute to the delivery of safe, effective, patient-centred care. We welcome applications from both newly qualified and experienced nurses and offer excellent opportunities for training and professional development.

The service operates across a 7-day working pattern.

Main duties of the job
  • To contribute to processes for service and quality improvement
  • Assess the physical and psycho-social needs of patients and deliver therapeutic interventions based on best available evidence in order to improve health outcomes.
  • To manage own case load, delivering and evaluating service delivery to patients with a variety of medical conditions and those delegated by case managers.
  • To plan interventions with the Case Manager and other team members using the common assessment process
  • To take part in the referral management process to ensure patients are treated by the most appropriate member of the team/service
  • To contribute to the development of protocols and procedures developed for clinical pathways

Person Specification
EDUCATION, QUALIFICATION & TRAINING
Essential
  • Registered Nurse/Awaiting NMC PIN

Desirable
  • Evidence of further continued professional development
  • Post qualification experience in relevant speciality
  • Hold / be working towards an NMC approved mentoring qualification

EXPERIENCE
Essential
  • Ability to understand the role of a registered nurse
  • Ability to work as part of a team and on own initiative
  • Awareness of current professional nursing issues

Desirable
  • Post registration experience working in an acute hospital setting

SKILLS, ABILITIES & KNOWLEDGE
Essential
  • Good verbal and written communication skills and ability to negotiate
  • Ability to manage and prioritise own caseload with supervision; Willingness to develop teaching skills
  • Ability to solve problems and use initiative
  • Ability to develop clinical skills
  • Willingness to undergo training (e.g. medications) Basic IT skills
  • Awareness of the NHSE guidance of clinical governance in order to continuously improve quality of care e.g. quality, safety and ward audit standards
  • Knowledge of the NMC code of conduct
  • Ability to maintain a safe clinical environment conducive to patient care
  • Adaptable to change
  • Ability to work rotating shifts
  • Diplomatic approach with professionalism
  • Flexible and approachable

Desirable
  • Knowledge of care planning, preceptorship, reflection and clinical supervision

SPECIAL CIRCUMSTANCES
Essential
  • Able to travel to other sites and off site as necessary
