Bramley, Wortley, Middleton (BWM) PCN is seeking a Clinical Pharmacist to join our well established PCN pharmacy team. This is an exciting opportunity to join an innovative and supportive multi-disciplinary team. The post holder will be a pharmacist, who acts within their professional boundaries, providing clinical support within the PCN and working as part of multi-disciplinary teams in general practice. In this role you will receive appropriate clinical supervision. You will ensure practices integrate with community and hospital pharmacy to help utilise the skills mix within teams, improve patient outcomes, ensure better access to healthcare and help manage workload. Perform a patient facing role to include undertaking clinical medication reviews to proactively manage patients with complex polypharmacy in a variety of settings including the surgery, patient homes and residential care, and with a view to meeting demands of new workstreams in primary care. Provide support to general practice staff with regards to prescription and medication queries, providing expertise in clinical medicines advice.

Main duties of the job
You will be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy and those with multiple co-morbidities. Require understanding of the principles of medicines optimisation throughout the PCN and practice teams, promoting good practice in line with national and local requirements.

Action clinical letters and discharge summaries, providing leadership on person centred medicine optimisation and quality improvement. Assist with drug monitoring including high risk drug monitoring, switching medications to agreed and approved protocols. Manage the repeat prescribing reauthorisation process, by reviewing requests for repeat prescriptions & medicines reaching review dates, minimising clinical risk and aiming to reduce wasted medicines.

BWM PCN compromises of four member practices with a joint patient population of 30,000 (approximately), each with its unique strengths and expertise. Our aim is to leverage these strengths and foster collaboration among our network members to deliver its key objectives contained in the Directed Enhanced Service Contract (DES) from NHS England and NHS Improvement and enhance the quality and efficiency of care delivery and by working together, we can offer a wide range of services, support and coordinated care effectively, and improve patient outcomes.

Job responsibilities
The post holder will provide support to meet the individual needs of Practices. Responsibilities may vary across Practices and can include:

• Repeat prescribing and medication reviews.
• Ensure patients have appropriate monitoring tests in place when required.
• Reduce inappropriate poly-pharmacy and wasteful prescribing and address public health and social needs of patients through clinical medication review. This would be a level 3 medication review looking at the patients full clinical condition e.g. blood monitoring, interface care arrangements, social isolation.
• Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
• Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related issues.
• Interface with community and hospital pharmacy colleagues and develop referral processes between primary care professionals including the promotion of the repeat dispensing service.
• Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.
• Manage Practice formularies to improve the quality, safety and cost effectiveness of prescribing.
• Work with primary care professionals and patients to implement NICE and other evidence based guidelines.
• Act as a source of medicines information for all of the Practice team and patients (e.g. around doses, side effects, adverse events, possible alternatives if stock issues).
• Provide education and training to primary healthcare team on therapeutics and medicines optimisation.

Qualifications
Essential

• Accredited masters degree of pharmacy Degree (MPharm) (or equivalent)
• Registered with General Pharmaceutical Council (GPhC)
• Completed, or working towards, a Primary Care Pharmacy Education pathway qualification (CPPE)
• Minimum of 2 years post qualification experience
• Experience of using System1.

Desirable

• Membership of the Royal Pharmaceutical Society
• Independent prescriber or working towards/intent of gaining an independent prescribing qualification

Experience
Essential

• Minimum of 2 years post qualification experience
• In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
• An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
• An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing.
• Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients).
• Can work independently on a day-to-day basis and able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long-term conditions.
• Excellent written and verbal communication skills (Email, telephone & online meetings).
• Evidence of consistent pattern of learning from education, training, and experience.
• Able to work under pressure and to meet deadlines.
• Good IT skills.
• Recognises priorities when problem solving and identifies deviations from the normal pattern and can refer to seniors or GPs when appropriate.

Desirable

• Able to analyse and interpret prescribing data
