Job summary
We are looking for a highly motivated, adaptable individual to join our team on the Discharge Lounge, Integrated Hub at Huddersfield Royal Infirmary

The successful candidate will need to be able to use their own initiative and work well within a team.

The discharge lounge is a busy, pro active ever changing environment, diverse and challenging .

Main duties of the job
  • Transferring and accepting patients from every ward area, to the discharge lounge into the integrated hub.
  • Coordinating discharge planning of the client, liaising with care providers, care facilities and family members. Recognising the need for further community referrals to safeguard patient safety and create an effective discharge .
  • Completing patient education with regards to care providers, the discharge plan, medication management and any follow up arrangements.
  • Effective communication and excellent documentation on the electronic patient record
  • Whilst in our care, completing hygiene, nutrition, hydration, elimination and mobility needs. Engaging with the patients throughout their time in the hub, offering comfort, compassion and reassurance.

We employ more than 6,500 staffwho deliver compassionate care from our two main hospitals,Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, healthcentres and in patients' homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.

We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children's and young people's services; end of life care and outpatient and diagnostic imaging services.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

We continue to modernise and invest in our health services to build on our strong reputation.Foundation trusts are public leaders in improving quality in health services. Job responsibilities
  • Leadership in discharge planning.

Qualifications & Training
Essential
  • NMC Registered Nurse in relevant field of practice
  • Practice Assessor/Practice Supervisor qualification or willingness to complete

Desirable
  • Additional post-registration qualification relevant to field of practice
  • Evidence of continuing professional development
  • Role specific training relevant to specialty of the post
  • Teaching qualification
  • Leadership qualification

Knowledge, Experience & Experitise
Essential
  • Knowledge and skill to plan, deliver, and evaluate high quality, evidence-based nursing care to a group of patients within the speciality of the post
  • Experience of working with patients, and those that matter to them, from a range of diverse cultures and backgrounds
  • Experience of working as part of a team, with the ability to prioritise own workload.
  • Knowledge and skill in infection prevention and control principles and procedures
  • Knowledge and skill in safeguarding principles and procedures
  • IT literacy and ability to use NHS systems
  • Competence in speciality or role specific extended skills required of the post or willingness to undertake necessary training

Desirable
  • Clinical experience in the specialty of the post
  • Evidence of introducing change and innovation into nursing practice
  • Skills in research and audit methodology and application of findings to practice

Communication & Relationships
Essential
  • Excellent interpersonal and communication skills with the ability to adapt to meet the requirements of patients, their families/carers, and professional colleagues.
  • Ability to work under pressure
  • Flexibility to meet the needs of the service

Desirable
  • Human Factors awareness
