Job summary
Cambridge City 4 PCN Ltd. will provide the right candidate with a supportive and nurturing environment with access to continued professional development and career progression opportunities.

Working across six Partner practices (registered list size of 59,000) and five aligned care homes, we employ a multidisciplinary team of Clinical Pharmacists and Pharmacy Technicians, Social Prescribers, Care Coordinators, Health Coaches, First Contact Physiotherapists and Paramedic.

Our current Pharmacy team consists of 4 full time Clinical Pharmacists and Independent Prescribers, 2 part time remote locum Clinical Pharmacists, and 8 Pharmacy Technicians (3 are currently remote locums) covering 7 whole time equivalents. Each of our 6 Partner practices has a dedicated "team of 2" this being a regular Clinical Pharmacist and supporting Technician. We come together as a team every Thursday at our PCN HUB which provides an opportunity for peer support, CPD, team building/bonding and meetings.

We are keen to enhance the team and our services to patients by increasing our pharmacy provision and offer hybrid and flexible working arrangements.

Main duties of the job
Working collaboratively with GP practice teams, PCN Clinical Directors, PCN Manager, Lead Clinical Pharmacist, Development and Operational Leads, and community pharmacy colleagues.

We are seeking an experienced or newly qualified Clinical Pharmacist who acts within their professional boundaries, supporting and working alongside a team of Pharmacists in general practice.

The successful candidate will work as part of a multi-disciplinary team in a patient-facing role, taking responsibility for areas of;
  • Chronic disease management
  • Clinical medication reviews
  • Proactive management of patients with complex polypharmacy

Providing primary support to general practice staff with regards to
  • Prescription and medication queries
  • Repeat prescription system
  • Acute prescription requests
  • Medicines reconciliation on transfer of care
  • Systems for safer prescribing
  • Providing expertise in clinical medicines advice
  • Addressing both public and social care needs of patients
  • Providing clinical leadership on medicines optimisation and quality improvement
  • Managing some aspects of the quality and outcomes framework and enhanced services.
  • Ensuring that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.

The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Job responsibilities
  • We are looking for an enthusiastic and patient-focused Clinical Pharmacist to join our established pharmacy team within Cambridge City 4 PCN.
  • The successful candidate will become an integral member of one or two GP practice teams, providing high-quality clinical pharmacy services while benefiting from the support of a large network pharmacy team consisting of clinical pharmacists and pharmacy technicians.
  • You will work closely with your designated pharmacy technician to deliver medicines optimisation, improve patient outcomes and support practices in achieving high-quality, safe and efficient prescribing.
  • This is an excellent opportunity for someone looking to develop within primary care, with strong mentorship, regular peer support and excellent professional development opportunities.

Person Specification
Qualifications
Essential
  • Masters degree in Pharmacy (MPharm)
  • GPhC registered
  • Undertaking or qualified CPPE training pathway
  • Proficient SystmOne user
