JET is part of the Urgent care Response , admission avoidance services and comprises of registered nurses and paramedics. You will be working with system partners to maximise capacity and deliver a 2 -4 hour response service to patients who have a GP registered within Cambridgeshire and Peterborough boundaries .

To support the JET Practitioners in ensuring the delivery of a safe and effective service. To look at alternative pathways to keep the patient at home, is clinically safe to do so .

Part of the role will be supporting the UEC [Urgent and Emergency Care Hub] as a Trusted assessor where you will be expected to clinically triage the referrals that come into the hub to ensure they are appropriate for services.

Main duties of the job
1; To provide a high quality of care to patients who have a wide range of physical needs affecting their ability to maintain a level of independence, with range range of 18 years and upwards with long term conditions.

2: To deliver patient centred care that empowers individuals to achieve their optimum physical, psychological, social and emotional wellbeing.

3: To provide holistic and clinical assessment, investigate, diagnose and implement patient centred care working in partnership with MDT.

4: To provide advice and support to patients, families, carers and voluntary agencies regarding medical condition ensuring that an effective communication pathway exists.

5: To provide a holistic person centred approach to care that meets the specific needs of the individuals by enabling an increase in their independence, improving health outcomes and reducing hospital admissions and long term care.

6: To ensure all nursing care is evidence based and of a high professional standard. To maximise the use of staff and equipment resources effectively.

• Ensure there are robust and strong lines of communication with GP, MDT, Community staff as well as the wider team
• Ability to communicate complicated and sensitive information in an appropriate manner to patients, families and carers, promoting understanding whilst recognising barriers to communication that might exist.
• Be aware of barriers to patients and carers understanding, due to intellectual, cultural or physical limitations and work to overcome these.
• To recognise potential breakdown and conflict when it occurs, seek advice and support and demonstrate established negotiation skills in the management of conflict.
• To deal with complaints sensitively avoiding escalation where possible, in line with the trusts policy on managing complaints and reporting the incident to the line manager.
• To be accountable for the shift as the senior practitioner, to ensure safety of patients and staff.
• Responsible for the mentoring and supporting the development of Band 6 practitioners.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
