The role includes undertaking home visits, managing urgent and same-day demand, and supporting patients over 65 with acute and long-term conditions. The post-holder will also be responsible for managing their own clinical workload, including referrals and results.

Main duties of the job
About us
Coastal Primary Care Network (PCN) is a partnership of four practices: Carnon Downs Surgery, Chacewater Surgery, Perranporth Surgery and St Agnes Surgery. Together, we provide high-quality, patient-centred care to our local population and are committed to developing innovative services that deliver the right care, in the right place, at the right time.
We are excited to be establishing a new Integrated Neighbourhood Team Frailty Service, created to provide proactive, coordinated support for people living with frailty across our communities. Working closely with GPs, community services, social care and voluntary sector partners, the service will focus on improving patient outcomes, supporting independence, reducing avoidable hospital admissions and helping people remain well in their own homes for longer.
This is a unique opportunity to join a brand-new service. The successful candidate will be involved in establishing and shaping the Frailty Team from the outset, helping to develop pathways, processes and ways of working that will support the long-term success of the service. As part of a MDT, you will contribute to service innovation, collaborative working and the delivery of high-quality care for some of the most vulnerable people. The post is offered on a fixed-term basis for and would suit an individual who thrives in a developing environment, enjoys building new services and is passionate about improving care for older people and those living with frailty.
