Job summary
CNWL is looking an enthusiastic and highly motivated Consultant to join the Kilburn Community Mental Health Team, based at Park Royal Centre for Mental Health. This would be an excellent opportunity for someone wanting to broaden their experience of community psychiatry and develop their clinical skills in a supportive and challenging environment. The team is well established with good support from senior clinical and managerial colleagues, and has opened since the current post-holder is relocating.

Main duties of the job
This is a full-time replacement post working within the Kilburn Locality Community Mental Health Team. The post holder will have consultant and RC responsibility for a defined caseload of service users under the care of the Kilburn Locality Community Mental Health Team. The post holder will also provide mentoring and supervision as an experienced Consultant.

Brent Adult Mental Health Service provides a range of services for adults within community and hospital settings. Our community services have recently transformed to improve the quality of care received by people with mental health needs in their local community in line with the NHS Long Term Plan, towards provision of more integrated, personalised and place-based care. Brent adult mental health services: Acute in-patients (3 wards - 47 beds) and PICU (11 beds) - Park Royal Centre for Mental Health; Community Mental Health Hubs - organised in three hubs serving the three GP networks in the borough. Harness and Kilburn hubs are based at Park Royal Centre for Mental Health, Park Royal; Kingsbury and Willesden Hub is based at Fairfields House, Kingsbury; Early Intervention Service is based at Fairlight Avenue; Home Treatment Team (HTT) is based at Park Royal; Mental Health Act Team (AMHP Team) is based at Park Royal; Park Royal Brent Integrated Psychology and Psychotherapy Services embedded within the community teams; Community Forensic Service is provided in collaboration with West London NHS Trust; Brent Talking Therapies; Brent Community Collaboration and health inequalities team; Mental health social team which deliver AMHP, hospital discharge, placements and complex case management; Rethink mental health team.

Job responsibilities
Principle Clinical Duties:

• Assessment of new patients and clinical supervision of assessments conducted by other team members.
• Full participation in the multidisciplinary team providing care in the community.
• Senior medical responsibility for the care conducted by the Community Mental Health Team for complex patients.
• Review and monitoring of the caseload to ensure appropriate care plan and packages are in place and discharges facilitated to maintain patient flow.
• Contribution to the continuous development of the Community Mental Health Team, and to other parts of the Service as required.
• Domiciliary visits to provide assessment and treatment services as a member of the CMHT.
• Domiciliary assessments in response to emergencies.
• Research, in terms of personal projects and clinical audits as part of clinical governance and quality improvement.
• Involvement in joint planning of future service development.
• Occasional psychiatric assessments of residents who may be in crisis in another borough.
• Close liaison with the wards to facilitate admission and discharge planning.
• Liaison with HTT, Psychiatric Liaison Services, AMHP services and Forensic Services in the borough.

Leadership Responsibilities:

• Leadership role in the quality agenda: identifying areas for improvement and active engagement with QI.
• Meet regularly with the team and local service managers to ensure quality, finance and performance targets are met.
• Support recruitment and ensure optimal retention of team members.
• Line management and clinical of junior and higher trainees and specialty doctors in the team.
• Teaching medical students.
• Engage in service development opportunities, Borough SMT and Borough CQM.
• Actively link and develop a relationship with the Primary Care Network(s) you are affiliated to, to respond to their developing needs and support the move towards more integrated care.

Teaching and training
To include:

• There is one GP trainee and one part time specialty doctor in the Harness team.
• An active role in the postgraduate teaching programme which consists of a local academic meeting on Wednesday lunchtime. This meeting includes a Journal Club and case presentations. This takes place at the inpatient site Park Royal Centre for Mental Health.
• We also have an SAS Lead onsite Dr Theo Mavrogiannidis, theodoros.mavrogiannidis@nhs.net and Trust wide Lead for SAS lead Dr Prathamesh Kulkarno, prathamesh.kulkarni@nhs.net.
• Teaching/training of medical students who regularly attend CMHT as part of their psychiatry experience, from Imperial/Brunel school of medicine. Support is also given to work experience students where applicable.
• Appropriately trained consultants would be encouraged to take up the role of Educational Supervisors for core and higher trainees from the St Marys training scheme in Psychiatry. Candidates willing to take up this role will be supported to access relevant training, and this will be included in Job Planning.
• Training and teaching of wider MDT staff with the community teams and other teams in the borough.

General duties

• To manage, appraise and provide professional supervision to junior medical staff as agreed between consultant colleagues and the medical director and in accordance with the Trusts personnel, policies and procedures.
• To ensure that junior medical staff working with the post holder, operate within the parameters of the New Deal and are Working Time Directive compliant.
• To undertake administrative duties associated with the care of patients.
• To record clinical activity accurately and comprehensively, and submit this promptly through the trusts clinical information systems.
• To participate in service and business planning activities for the locality and, as appropriate, for the whole mental health service.
• To participate in annual appraisal for consultants.
• To attend and participate in the academic program of the Trust, including lectures and seminars as part of the internal CPD program.
• To maintain professional registration with the General Medical Council, Mental Health Act Section 12(2) approval, and to abide by professional codes of conduct.
• To participate annually, as a minimum, in a job plan review with the clinical manager, which will include consultation with a relevant manager to ensure that the post is developed to take into account changes in service configuration and delivery associated with modernization.
• To work with local managers and professional colleagues in ensuring the efficient running of services and share with consultant colleagues in the medical contribution to management.
• To comply with the Trusts agreed policies, procedures, standing orders, and financial instructions, and to take an active role in the financial management of the service.

Qualifications
Essential

• MB BS or equivalent medical qualification

Desirable

• Qualification or higher degree in medical education, clinical research or management
• MRCPsych OR MRCPsych equivalent approved by the Royal College of Psychiatrists
• Additional clinical qualifications

Eligibility
Essential

• Fully registered with the GMC with a licence to practise at the time of appointment
• Included on the GMC Specialist Register OR within six months
• Approved clinician status OR able to achieve within 3 months of appointment
• Approved under S12 OR able to achieve with 3 months of appointment

Desirable

• In good standing with GMC/RCPsych with respect to warning and conditions on practice

• Holds and will use valid UK driving licence OR provides evidence of proposed alternative
