Main duties of the job
The post holder will be a qualified Health Visitor. You will work autonomously as part of the Integrated Early Years Service (Health Visiting & Family Support) managing a designated caseload of children and families, in order to improve their health and enable them to reach their full potential.

You will work collaboratively with other agencies to provide high quality care to children and families. Child protection work will involve gathering and analysing information to make informed assessments, appropriate referrals and participating and contributing at case conferences and core group meetings.

You will support public health initiatives in line with local and national priorities which promote, protect and improve the health of the population and reduce health inequalities.

Job responsibilities
Health Visitors contribute to the provision of safe and reliable services by:

• Using their clinical judgement and risk assessments to keep the people using our services as safe as possible
• Safeguarding people by recognising and responding when an adult or child might be at risk from abuse but also recognising their own limits and asking for help and escalate concerns when necessary.
• Escalating safety concerns and by doing so acting as effective advocates for those who use our services
• Being open and transparent about their own practice.
• Supervising the work of others.
• Reflecting on everyday practice to identify areas where improvements in safety or quality can be made.
• Working with others to create a culture of continuous improvement.
• Maintaining accurate, legible, comprehensive records.
• Maintaining compliance with their mandatory training requirements.

Health Visitors often work with children and families who have complex needs and contribute to the effectiveness of services by:

• Providing skilled, evidence based care which adheres to agreed policies and procedures.
• Working with children and families in all stages of the care planning process including assessing risks and needs.
• Working as autonomous practitioners and taking responsibility for the care they give to children and families but also work as team members.
• Contributing to creating and maintaining high performing teams by:
• communicating well with all members of the team
• understanding their role in the team and how they help the team achieve its objectives.
• reflecting on their own practice regularly and encouraging the whole team to reflect on their practices in team meetings
• encouraging the team to learn from adverse events or respond to data analysis from audits or benchmarking exercises and implement sustainable initiatives which improve clinical outcomes

Person Specification
Skills and Knowledge
Essential

• Able to communicate with the multi-disciplinary team, present cases, take note of actions, update others in the team and use handover effectively. Speaking face to face but also by telephone and writing letters and reports.
• Maintaining accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the CNWL policies and be competent in using Word, Teams, Zoom, Outlook and other electronic systems
• Work well with other members of the team including communicating effectively, being kind and considerate, reliable and enthusiastic
• Mentor, teach and support others in the team

Desirable

• Supervision and appraisal training.

Qualifications
Essential

• Registered Nurse [NMC Registered]
• Registered Specialist Practitioner Public Health Nurse (Health Visitor)
• Evidence of relevant Continuous Professional Development

Desirable

• Mentorship Preparation Module/ ENB 998/ Practice Assessor
• Degree or Master's level study in a relevant field (e.g. Public Health, Child Health)
• Nurse Prescriber qualification (V100/V300)

Previous Experience
Essential

• Experience of working with children, families, and/or vulnerable groups
• Ability to work autonomously while recognising limits and seeking support appropriately.
• Knowledge of safeguarding children, including current legislation and local procedures
• Experience of delivering the Healthy Child Programme
• Experience of multi-disciplinary and multi-agency working
• Knowledge of infection prevention and control standards
• Knowledge of risk assessment and risk management
• Awareness of current public health priorities (e.g. health inequalities).
• Knowledge of information governance and confidentiality requirements (e.g. GDPR).
• Experience in community settings
• Ability to work flexibly and respond to changing service demands and methods of working both individually and within a team.
