Job summary
The Home First Service supports the timely and safe discharge of patients from acute and urgent care settings, in line with national NHS priorities.

Working within a multidisciplinary team, the service provides rapid assessment and short-term, intensive interventions for patients with complex health and social care needs. The focus is on promoting independence, recovery and person-centred care within the community, helping individuals remain safely at home wherever possible.

As a Band 6 Occupational Therapist, you will manage a complex caseload, contribute to clinical decision-making, and play a key role in coordinating care within a fast-paced and dynamic environment.

The service works closely with acute trusts, primary care, community services across CLCH, and local authority social care teams to deliver integrated, high-quality care and improve patient outcomes.

The service operates 7 days a week, from 8:00am to 8:00pm, including bank holidays. Full-time staff are expected to work flexibly across a 7-day rota, including early, late and long-day shifts as required.

Main duties of the job
  • Act as an active member of the multidisciplinary team, delivering a comprehensive Occupational Therapy service to residents referred to the Brent Home First Service.
  • Provide specialist Occupational Therapy assessments and interventions that support safe and effective discharge planning.
  • Facilitate timely assessments and coordinated discharge planning in collaboration with colleagues across health and social care services.
  • Manage your own caseload independently, working with appropriate levels of clinical supervision.
  • Support the supervision and development of Band 5 Occupational Therapists, Occupational Therapy students, and support staff.
  • Contribute to service delivery across different teams or units as required, demonstrating flexibility to meet service needs.
  • Promote person-centred care, independence, and positive outcomes for patients receiving Home First interventions.

Person Specification
Education/Qualification
Essential
  • Degree/Diploma in Occupational Therapy
  • Current AHP with UK HCPC registration

Desirable
  • Post graduate training

Experience
Essential
  • Post qualification experience which should include: Elderly Care. Older people's Rehabilitation. MDT working. Falls prevention.

Desirable
  • Cognitive assessments.
  • Intermediate Care/Community Rehabilitation.

Skills & Knowledge
Essential
  • Good communication skills.
  • Able to work as part of a team.
  • Demonstrate a problem solving approach.
  • Able to delegate work and supervise staff
  • Student supervision/teaching skills.
  • Able to prioritise and manage own time
  • Able to present information both written and verbal.
  • Aware of current issues in Occupational Therapy/ Care/NHS.
  • Awareness of the legal responsibilities of the profession.
  • IT skills

Key Attributes
Essential
  • Evidence of involvement in service delivery improvement.
  • Hold a full, valid, UK driving licence and have access to a car to use for business purposes
