An exciting opportunity has arisen for a
Band 3 Community Support Worker to join our
Brent Urgent Community Response (UCR) Team at Central London Community Healthcare NHS Trust.
This is a
full-time role, working 37.5 hours per week, supporting the delivery of high-quality care to patients within their own homes and community settings.
Working as part of a supportive multidisciplinary team, you will help provide timely care and support to patients who may be experiencing a change in their health or circumstances. You will play an important role in helping patients maintain their independence, supporting their recovery and
preventing avoidable hospital admissions wherever possible.

Band 3 Community Support Worker within the Brent Urgent Community Response Team, you will:
- Provide safe, compassionate and personalised care to patients in their own homes, working alongside registered nurses, therapists and other members of the multidisciplinary team.
- Support patients with a range of nursing and therapy needs, helping them to maintain their independence and remain safely at home wherever possible.
- Carry out delegated clinical activities and interventions under the direction of a registered healthcare professional and within your agreed level of competence.
- Provide holistic, person-centred support, recognising changes in a patient's needs or condition and reporting concerns appropriately.
- Support the team with relevant clinical and administrative duties to ensure patients receive timely and coordinated care.
- Communicate effectively and sensitively with patients, families, carers and healthcare professionals.
- Maintain accurate patient records and confidently use the relevant IT systems required for the role.
- Work collaboratively as part of the wider community team to deliver a responsive and effective service.
Please note: A
full UK driving licence and access to a car for work purposes are essential for this community-based role.

QualificationsEssential- Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3)
- A recognised equivalent course
- Willingness to undertake further training as appropriate
Desirable- GCSE grade A-C or equivalent in English and Maths
- ECDL certificate
- Registration with the national regulatory body (when it becomes available).
- PSI qualification
ExperienceEssential- Experience of working in a health or social care setting
- Experience of working as part of a team
- Experience with the use of Microsoft Office and a variety of databases.
Desirable- Experience of working as part of a team
- Experience with the NHS patient information database (SystmOne, Rio, CIS)
Skills & KnowledgeEssential- Knowledge of NHS An awareness of risk management.
- Demonstrate an acceptable level of numeracy and literacy.
- Knowledge of multiagency services available and processes for referral
- Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
- Self-management skills-managing time and workload
- Ability to analyse, interpret and record data in a clear, and succinct manner
- Good analytical and problem solving skills
