Job summary
An exciting opportunity has arisen for an experienced and motivated Band 8C Frailty Consultant Practitioner. To provide highly specialised clinical expertise and leadership across the Hospital at Home and Urgent Community Response services within Merton and Wandsworth.

The Frailty Consultant Practitioner will hold clinical accountability for a defined proportion of the community caseload and provide expert clinical leadership in the assessment, treatment and management of acutely unwell and frail patients within the community.

The postholder will lead the development and delivery of an integrated community-based Urgent Neighbourhood Health service, bringing together Hospital at Home and Urgent Community Response and working across community health, acute partners and wider system providers. They will support the delivery of safe, effective and responsive care closer to home, reducing avoidable hospital attendance and admission and facilitating timely discharge where appropriate.

The post will be structured abound the four pillars of advanced clinical practice:

• Expert clinical practice (minimum 60%)

• Education, training, and development
• Research, evaluation, and service development

Main duties of the job
The Frailty Consultant Practitioner day-to-day duties will include:

• Providing clinical leadership to the Urgent Neighbourhood Health team, encompassing Hospital at Home and Urgent Community Response.
• Practising autonomously at an advanced level, using highly developed clinical assessment, diagnostic, decision-making and treatment skills, with access to defined medical supervision and escalation arrangements.
• Providing clinical leadership and oversight of the Single Point of Access for urgent community health needs, supporting timely clinical triage, navigation and decision-making to ensure patients are directed to the most appropriate pathway and level of care.
• Holding clinical accountability for a defined caseload of patients and providing expert clinical oversight, advice and decision-making for patients with complex, acute and frailty-related needs.
• Providing expert clinical advice and consultancy to multidisciplinary professionals, individuals, families and carers.
• Providing visible clinical leadership across the multidisciplinary team, promoting evidence-based, person-centred care, treatment and support.
• Develop new and innovative clinical pathways in the community that shift care from the hospital into the community.

Qualifications
Essential

• Registered on the NMC or HCPC register
• MSc in relevant subject field
• Independent prescriber
• Evidence of a significant CPD portfolio, related to Frailty
• Leadership training suited to clinical leaders

Desirable

• PhD
• Teaching qualification
• Management/ Leadership qualification

Experience
Essential

• Experience working at band 8a level or equivalent within frailty
• Advanced clinical knowledge related to frailty
• Highly effective Multidisciplinary working
• Evidence of service development with key stakeholders
• Evidence of implementing innovative practice
• Evidence of leading successful change management
• Participation in research studies
• Mentor/ Educator; Significant experience of teaching and assessing clinical practice
• Substantial line management experience, including providing clinical supervision and appraisals
• Financial management of a service budget

• Experience of working with national professional bodies within specialty area to implement change
• Experience in recruitment, retention, and staff performance
• Publication of research

• As this is a community-based role, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
