Job summary
Central Medical Centre is seeking an Advanced Nurse Practitioner (ANP) to provide autonomous, evidence-based clinical care within their scope of practice to our registered patients within a busy general practice setting.

The post holder will assess, diagnose, treat, prescribe and manage patients across all age groups while working collaboratively with the multidisciplinary team to deliver high-quality, patient centered care.

This role offers an exciting opportunity to provide autonomous clinical care within a supportive general practice environment, contributing to improved patient outcomes and high-quality primary care.

The successful candidate would ideally have experience in managing patients with diabetes and will play a key role in delivering high-quality diabetes care, supporting long-term condition management and improving patient outcomes.

The role also involves leadership of the nursing team, which includes practice nurses and HCAs and leadership at a practice level of nursing related clinical workstreams.

Main duties of the job
Responsibilities of the role include:-

• Independently assess, diagnose, treat and manage patients presenting with acute and chronic illnesses.
• Undertake comprehensive history taking, physical examinations and clinical assessments.
• Request, interpret and act upon appropriate investigations.
• Implement and evaluate evidence based treatment plans.
• Undertake home visits where appropriate.
• Support the delivery of diabetes care within the practice including conducting annual diabetic reviews in accordance with national guidelines. Optimise glycemic control through medication and lifestyle interventions alongside management of overall cardiovascular risk. Initiate and adjust diabetes medications, including insulin where appropriately trained and within scope of practice. Monitor and manage complications associated with diabetes in line with scope ofof practice.

Central Medical Centre is a well established forward thinking training practice located in Morden, Surrey.

The practice is situated close to Morden town centre, which has good public transport links including the London Underground Northern line and multiple bus routes.

The practice strives to provide high quality care to approximately 9700 patients in the local community and surrounding areas and is based over 3 floors.

We have an exciting opportunity for someone to join our multi disciplinary team to support our patients and clinicians. Our multi disciplinary team of 30 staff includes GPs, Practice Nurses, Clinical Pharmacists, HCA's, Mental Health Practitioner and Social Prescriber all of who are supported by an our non-clinical team that includes Patient Care Co-Ordinators, Receptionists and Administrators. The team work closely together to support each other in a friendly and welcoming atmosphere.

The practice is fully computerised and uses EMIS as it's clinical system.

Job responsibilities
Primary responsibilities
The following are the core responsibilities of the Advanced nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks including routine nursing. This will be dependent upon factors such as workload and staffing levels:

• Independently assess, diagnose, treat and manage patients presenting with acute and chronic illnesses.
• Undertake comprehensive history taking, physical examinations and clinical assessments.
• Request, interpret and act upon appropriate investigations.

• Manage urgent presentations, ensuring timely escalation where necessary.
• Undertake home visits where appropriate.
• Maintain accurate and contemporaneous clinical records ensuring that snomed codes are used effectively
• Support the delivery of diabetes care within the practice including conducting annual diabetic reviews in accordance with national guidelines. Optimise glycaemic control through medication review and lifestyle interventions alongside management of overall cardiovascular risk.
• Initiate and adjust diabetes medications, including insulin where appropriately trained and within scope of practice.
• Monitor and manage complications associated with diabetes in line with scope of practice.
• Support patients with education on self-management, diet, exercise and medication adherence.
• Work proactively to improve diabetes outcomes and support QOF performance and enhanced service delivery.
• Collaborate with the wider multidisciplinary team, including community diabetes services and secondary care specialists.
• Manage and review patients with long term conditions including Diabetes, Hypertension, Asthma, COPD, Cardiovascular disease, Chronic Kidney Disease, Heart failure - implementing and evaluating individual treatment plans for chronic disease patients.
• Provide holistic care aimed at prevention, early intervention and optimisation of treatment including promoting healthy lifestyles and preventative healthcare.
• Deliver personalised care planning and encourage self-management and shared decision making.
• Support national screening and vaccination programmes.
• Undertake medication reviews.
• Monitor effectiveness and adverse effects of treatments in line with scope of practice.
• Promote safe and cost-effective prescribing in line with scope of practice.
• Participate in clinical audit and quality improvement initiatives.
• Support implementation of national and local clinical guidance.
• Participate in significant event analysis and clinical governance activities.
• Communicate effectively with patients, relatives and carers.
• Work collaboratively with GPs, nurses, pharmacists, healthcare assistants and administrative staff.
• Liaise and refer patients appropriately to secondary care, community services and other health and social care partners where required.
• To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health.
• To provide routine nursing care to patients as required in accordance with clinical based evidence.
• To process pathology results as required and where within scope of practice and competencies.
• Support members of the HCA/nursing team, providing guidance, leadership and mentorship when necessary.
• Participate in local initiatives to enhance service delivery and patient care.
• Support and participate in shared learning within the practice.
