This is an excellent opportunity for an energetic and passionate Advanced Nurse Practitioner to help shape our vision in how we provide clinical services to individuals whose lives have been affected by drugs and alcohol.

Main duties of the job
The Advanced Nurse Practitioner Homelessness Outreach will provide advanced, autonomous clinical care to individuals experiencing homelessness across West Sussex, with a particular focus on addressing health issues related to substance misuse and alcohol dependency. Working flexibly between service hubs and outreach settings, the post holder will deliver comprehensive health assessments, urgent primary care including wound care/immunisations/BBV treatment pathways, and ongoing management of substance and alcohol related health conditions alongside coexisting physical and mental health needs. The role requires independent clinical decision making, nonmedical prescribing, and close collaboration with multiagency partners to respond effectively to the complex and often interrelated health and social needs associated with homelessness. Reducing health inequalities and improving access to timely, appropriate healthcare for this underserved population is a central priority of the role.

Job description

Job responsibilities

Clinical Care
  • Deliver comprehensive health assessments and urgent primary care to individuals experiencing homelessness.
  • Diagnose, treat, and manage acute and chronic conditions independently within scope of practice and role.
  • Opportunistic immunisations including Hepatitis B schedule and Flu vaccines according to criteria for the respective PGDs.
  • Identifying substance and alcohol dependency working alongside national clinical guidelines and county pathways.
  • Discussing cases within MDT pathways in wider teams.
  • Follow the principles of the National Wound Care Strategy Programme (NWCSP). Identify the most appropriate evidence-based practices as per the organisational wound care policy.

Prescribing
  • Prescribe medications and treatments in line with ANP qualifications and role remit, governance arrangements and protocols that are in line with organisational clinical pathways and the national formulary.

Multi-Agency Working
  • Work closely with statutory housing teams, mental health services, social workers, and Wsx substance misuse hubs/teams.

Inclusion Health
  • Provide harm reduction support, health promotion advice. Facilitate access to mental health services.
  • Collaborate with Wsx wider service hubs for engagement with sub-mis teams for ongoing case management.

Care Coordination
  • Support holistic care planning and signpost individuals to appropriate ongoing health and social care services.
  • Develop therapeutic alliances with the homeless population and develop effective partnerships with them to support their individual health needs, offering health promotion and harm reduction advice
  • Work closely with Wsx front line homeless health outreach teams to ensure clear lines of responsibility of care.

Clinical Records
  • Maintain accurate, timely and contemporaneous clinical records using CGL electronic record systems, CriiS.

Safeguarding
  • Uphold safeguarding responsibilities and ensure patient safety, dignity and professional boundaries are maintained.

Service Development
  • Contribute to service development through data collection, audit, evaluation and quality improvement activity.

Person Specification

Qualifications

Essential
  • Registered Nurse with current NMC registration.
  • Qualified Advanced Nurse Practitioner (masters level or equivalent).
  • Independent Prescribing qualification (V300).
  • Experience in community health, primary care, urgent care/A&E or homeless health services.
  • Strong advanced clinical assessment, diagnostic and decision-making skills.
  • Understanding of the complex health and social needs of people experiencing homelessness.
  • Knowledge of safeguarding procedures and public health and inclusion health frameworks.
  • Resilience, empathy and excellent communication skills.
  • Full UK driving license and willingness to travel across West Sussex.

Desirable
  • Experience working with mental health, substance misuse, and co-occurring conditions.
  • Familiarity with voluntary and statutory services supporting people experiencing homelessness in West Sussex.
