Channel Health Alliance is seeking an experienced and motivated General Practitioner (GP) to provide clinical leadership within our Integrated Neighbourhood Team, delivering high quality, patient-centred care to patients across our Primary Care Network (PCN).

This is an exciting opportunity for a GP who enjoys autonomous practice, multidisciplinary working, and supporting the development of innovative community-based services. You will play a key role in providing senior clinical decision-making, leading clinicians, and ensuring timely, effective care for some of our most vulnerable patients.

Main duties of the job
About the Role
As the Integrated Neighbourhood Team GP, you will:

• Provide senior clinical triage and determine the most appropriate response to home visit requests.
• Deliver clinical leadership, mentorship, and support to Advanced Clinical Practitioners and the wider multidisciplinary team.
• Undertake home visits for patients with complex medical needs when required.
• Assess, diagnose, and manage patients in their own homes.
• Prescribe medication according to national and local prescribing guidance.
• Carry out CGA, RESPECT and SMRs.
• Support service improvement, education, training, and clinical governance activities.
• Work collaboratively with GP practices, care homes, community services, and partner organisations.
• Ensure high standards of patient safety, record keeping, and professional practice.

Channel Health Alliance was established in April of 2017 providing a range of primary and community services on behalf of the NHS. We are delighted to have attained accreditation with the Care Quality Commission, and a CQC rating of good which only a few federations have achieved.

Channel Health Alliance has embraced the continuing challenges faced by health and social care which has enabled us to provide high quality, innovative services for our diverse population.

Total Health Excellence (THE) East and West Primary Care Networks (PCNs) work with our partners to provide proactive, personalised care for people close to home. We operate across the Dover and Folkestone areas and eight GP practices form our PCNs. Our PCNs are led by a Clinical Director, who is a GP, supported by a team of around 50 highly qualified health professionals.

Job responsibilities
To provide clinical leadership and senior decision making for the integrated neighbourhood teams, ensuring high quality, timely and patient-centred care for housebound patients across the PCN population. The post holder will clinically lead Advanced Clinicians and the wider multidisciplinary team through triage, assessment, treatment planning, and service development.

Essential

• Medical Degree
• Full GMC Registration
• Registered on the Performers List
• Applicants must have current UK professional registration.

Desirable

• Clinical Supervisor/Educator

Experience
Essential

• Previous experience in primary care, or home healthcare preferred.
• Experience of leading a team of senior clinicians
• Experience of collaborating with multidisciplinary teams
• Experience of working with patients living with frailty, or a willingness to develop skills in this area
• Experience of education and mentorship of senior clinical colleagues
