Description

Job summary
We are looking for a caring, enthusiastic and experienced Practice Nurse (RGN) to join our friendly team. This is an important role within the practice, providing high-quality nursing care and helping our patients stay healthy, manage their conditions and access the support they need.

You will work closely with our GPs and wider healthcare team (Nurse, HCAs) to provide a safe, effective and patient-focused service. The role offers the opportunity to build strong relationships with patients while delivering a wide range of nursing services, from routine care and health checks to managing long-term conditions.

Main duties of the job
As a Practice Nurse, you will:
  • Provide professional nursing care in line with the NMC Code of Conduct and practice guidelines.
  • Assess, treat and support patients independently, referring to GPs or other professionals when required.
  • Support patients with long-term conditions through reviews, monitoring and personalised care plans.
  • Deliver preventative healthcare, screening and health education to promote patient wellbeing.
  • Maintain accurate and timely patient records.
  • Maintain organised clinical rooms, equipment and stock levels (to support HCA Team when needed)
  • Support health promotion activities and patient information initiatives.
  • Participate in practice meetings, teamwork and service improvements.
  • Deliver a range of clinical services, including: Cervical screening, Wound assessment, wound care and suture removal, Routine vaccinations and immunisations, including childhood immunisations, Travel health assessments, advice and vaccinations, Urinalysis, Flu vaccinations, Chronic disease reviews and ongoing management, Vaccine fridge temperature checks and cold chain compliance, Legionella and other relevant health and safety checks, escalating concerns appropriately.

City Square Medical Group is a busy inner-city GP practice located in Tower Hamlets, providing safe, effective and patient-centred healthcare to over 12,000 local residents.

We are a friendly and supportive team consisting of four GP partners and two salaried GPs, operating across two sites following our merger in April 2020. We are committed to delivering high-quality primary care and continuously improving the services we provide to our patients.

We are looking to recruit an additional Practice Nurse to join our growing team. This is an exciting opportunity for an enthusiastic and experienced nurse to become part of a collaborative multidisciplinary team and play an important role in supporting the health and wellbeing of our diverse patient population.

Person Specification
Qualifications
Essential
  • 1.Registered Nurse (Adult) with current NMC registration.
  • 2.Previous experience working in General Practice.
  • 3.Experience of managing long-term conditions.
  • 4.Competent in general treatment room procedures.
  • 5.Strong clinical assessment and decision-making skills.
  • 6.Excellent communication and interpersonal skills.
  • 7.Commitment to delivering high-quality, patient-centred care.

Desirable
  • 1.Experience supporting care home patients.
  • 2.Experience in vaccination and immunisation services.
  • 3.Experience with cervical screening.
  • 4.Travel health qualification.
  • 5.Experience using EMIS Web or similar clinical systems.
