Main duties of the jobThis is a community-based role that may require frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.
- To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.

Person SpecificationEducation and QualificationsEssential- Current professional registration - NMC
- Professional qualification - RMN
- Post graduate qualification or relevant experience
ExperienceEssential- Extensive experience of working with mental health patients
Knowledge and abilitiesDesirable- Knowledge around suicide awareness and prevention and the ability to support patients
