Job summary
We have an exciting opportunity for a motivated band 6 paediatric nurse to join our team as a band 6 Childrens Community Nursing Sister. The team provide nursing care to children and young people across County Durham and Darlington with a range of nursing needs. The service provides community care 7 days a week 08.00 to 20.00 and we have 2 office bases in Darlington Memorial Hospital and Chester-le Street Hospital. The successful candidate would be predominantly based in Darlington, however there would be an expectation that the post holder may need to work from either base to meet the needs of the service. This post is advertised for 34.5 hours per week.

Main duties of the job
You will work as a team of Children's Community Nursing Sisters, Staff nurses and healthcare assistants. The successful candidate will be a caseload holder for an allocated group of patients to ensure all their nursing needs are met. You will provide a high standard of nursing care to children and their families in a variety of community settings. This will involve carrying out visits to family homes, schools, nurseries and short break provisions to ensure the child's nursing needs are met. You will work autonomously as a lone worker whilst on visits in the community. Training will be provided to ensure that you have the necessary skills to deliver evidence based care and you will support junior staff to develop their own skills. As a band 6 Sister we are looking for a dynamic, innovative individual who is passionate about delivering the highest level of patient care. You must be flexible to meet the needs of the service and a team player.

About us

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes. Job responsibilities
The Children's Community Nursing Team have a varied caseload of children and young people, some of whom have complex health needs. In order for these children to access education and short break provisions you will coordinate and participate in delivering education to carers. Children and Young people are often discharged home from the acute setting to continue their care at home for a period of time. You will therefore be expected to coordinate a safe discharge home and administer IV medications and carry out other nursing care to meet the child's needs in the community. As a band 6 Sister you will be expected to coordinate the care being delivered by the team on allocated shifts. This will require an understanding of the wider patient list, the role and geographical area covered by the service. Due to the nature of the role you will be required to travel across County Durham and Darlington; you must have your own vehicle in order to travel as part of the role. You will be expected to start and finish your working day from your allocated base.
