Description
Job summary
An exciting developmental opportunity has arisen to recruit a MH nurse, occupational therapist or social worker with existing community experience. This is to work in our multi-disciplinary early intervention in Psychosis team as a Band 6 Care coordinator. This is a permanent WTE post.
This is a specialist community team so there is an expectation that you have substantial experience of working within a community mental health setting. Early intervention in Psychosis is a dynamic, innovative and evidence-based model that works within the remit of national targets.
The role involves detailed initial assessments incorporating the CAARMS assessment to identify 'At Risk Mental States' (ARMS) or first episode psychosis (FEP). This is incorporated into a formulation to plan the clients care over the next 3 years. The team uses a Recovery philosophy to collaboratively identify the individual's goals and needs and to support them towards achieving these. Many of our people experience difficulties with housing, finances, vocational activities and social inclusion and the successful applicant will be expected to work creatively, both with other team members and independently to support people to achieve their goals. We aim to support people to independence so they no longer need input from Mental Health Services.
Main duties of the job
The successful candidate will manage a case load plus contribute to the management of cases as an MDT. You will support the delivery of NICE interventions and the access and waiting time standard, provide psycho-therapeutic interventions such as CBA, BFT, psycho education, carers groups and early warning signs to assist patients and carers in the management of their illness.
You will need to be positive and enthusiastic in what can be a very demanding role, work independently and as part of the team. You will need a strong working understanding of CPA and maintain this to a good standard, having strong IT skills. A significant element of the role will be the identification and management of risk and positive risk.
You will need to demonstrate values that support recovery and maximise the potential of individuals, reduce social exclusion, stigma and the potentially disabling effects of psychosis. This will be achieved by quality engagement and care coordination.
You will require leadership skills to contribute to the day to day function of the team and development of the service. You need to be dynamic, enthusiastic and motivated to work within the EI model. You will need to have excellent interpersonal and problem-solving skills. You will be supervising support staff and students.
You need post-registration experience of working with adults and young people within a similar setting. The ability to travel and work flexibly with the needs of the our people is essential.
