Job summary
In Reach Acute Practitioner
This is a cross county post, covering Coventry and North and South Warwickshire, within the community Older Adults Team working with functional mental illness and dementia.

This post supports our Hospital at Home Pathway which is a 7 day a week service working between the hours of 08.00-20.00.

The role of in reach acute practitioner will work in the community Dementia and Older adults and the Hospital at Home pathway providing in-reach into acute hospital settings to providing a comprehensive assessment and liaison within Accident and Emergency and Acute wards across the County and beyond where indicated.

The In Reach Acute practitioner will provide formulation and care planning advice for older adults presenting with mental health difficulties. Working closely with mental health liaison and acute colleagues to ensure early identification of mental health problems and their appropriate management.

Main duties of the job
The In Reach Acute Practitioner will assess and triage patients and support other community staff in the triage and assessment of individuals who present with a range of age-related mental health problems including dementia.

The In Reach Acute Practitioner will be working in collaboration with other stakes holders who specialise in Older Adult care.

The In Reach Acute Practitioner will work closely with the Hospital at Home team adopting a step-up and step-down approach to reduce admissions to both General and Mental Health hospital.

The role will align with expectations for moving service provision from hospitals to the community, as well as with local and national ambitions for integrated neighbourhood teams. Place-based partnerships and integrated neighbourhood teams promote a focus on prevention and integration of statutory and voluntary agencies.

Coventry and Warwickshire Partnership Trust provides a full range of expanding physical, mental health and learning disability services to children, young adults, adults and older adults over many sites across Coventry, Warwickshire and Solihull. Services are provided to a population of over one million people living in Coventry and Warwickshire and also a wider geographical area in some of our specialist services, we see on average around 5,000 patients every day.

Job responsibilities
As the successful candidate you must have a professional qualification i.e., nursing, occupational therapist or social worker, with substantial post registration experience as a qualified professional of which you will have recent experience working with clients within a secondary care mental health setting.

Communicate with both internal and external stakeholders, such as service users, carers, other teams, third sector and voluntary organisations on matters which may be complex and/or contentious.

Communicate effectively across a wide variety of settings, using tact and diplomacy in response to situations that may be complex and sensitive and may have resulted from conflict.

Providing advice and use their initiative and follow organisational procedures in routine and crisis situations.

Apply communication skills which take account of other communication needs and differences, in order to plan, deliver and evaluate care.

Apply enhanced communication skills both oral and written to communicate information within records and reports within their service area. This may also include the need to respond to and offer and reassurance to questions and concerns from a variety of groups, such as service users, carers and other staff/teams.

Comply with and implement local and Trust policies and procedures regarding appropriate communication.

Be required to lead client reviews within the multi-disciplinary team and act as an advocate for service users.

Analyse and act on multiple and complex information assuring adherence to local and national guidance, legislation and policy.

Act within the limits of their competency and authority in line with their professional code of conduct.

Will manage and organise competing and conflicting priorities.

Review, evaluate and adjust activities in response to changing demands.

Have the ability to use I.T. systems sensibly and purposefully to meet the needs of the role and will be required to utilise systems safely and securely in line with organisational guidelines.

Attend annual training and undertake life support training appropriate to role.

Person Specification
QUALIFICATIONS
  • Relevant Professional Qualification i.e RMN, DipCOT
  • Current unrestricted registration with HCPC/NMC
  • Professional knowledge acquired through degree/ diploma supplemented by specialist training/experience, short courses to masters level equivalent
  • Evidence of continuing professional development

KNOWLEDGE & SKILLS
  • Excellent written communication skills

EXPERIENCE
  • Extensive knowledge of Clinical Governance, safety and quality measures and Clinical Supervision
  • Be able to prioritise and manage conflicting demands

OTHER
  • Must have full driving licence and access to a motor vehicle for business use
