Job summary
The Coco PCN Home Visiting Paramedic is an exciting opportunity for an experienced and motivated Paramedic to make a meaningful difference to patients across the local community.

Working across patients homes and GP practices, you will provide high-quality, compassionate and patient-centred care, using your clinical expertise to assess, diagnose, treat and support patients with a wide range of health needs. You will work closely with GPs and the wider multidisciplinary team to ensure patients receive the right care, in the right place and at the right time.

This role would suit someone who thrives on making a difference, is passionate about improving patient outcomes and enjoys working collaboratively with colleagues across primary, community and wider health and care services. Main duties of the job
Key Responsibilities

• Undertake comprehensive clinical assessments, examinations, diagnosis and treatment.
• Provide high-quality home visits to patients unable to attend the practice.
• Develop and review individualised care and treatment plans, including appropriate investigations and follow-up.
• Recognise and appropriately manage acutely unwell or deteriorating patients, escalating care when required.
• Work collaboratively with GPs and the wider multidisciplinary team to provide coordinated care.
• Support patients and families through clear communication, education and advice.
• Make appropriate referrals and onward care arrangements.
• Maintain accurate and timely clinical records.
• Participate in professional development, training and quality improvement.
• Work within all relevant clinical, safeguarding, confidentiality, health and safety and professional standards.

COCO PCN is a collaborative partnership between three GP practices in West Surrey & Sussex: Chertsey Health Centre, Ottershaw Surgery and Crouch Oak Family Practice. We are a forward-thinking, community-focused PCN committed to delivering joined-up, proactive and personalised care closer to home.

Our multidisciplinary team provides services including mental health support, paramedic home visiting, cancer care support and care-home coordination, working closely with local health and care partners to improve patient outcomes and experience.

Job responsibilities
Job Summary
We are seeking a skilled, compassionate and motivated Paramedic to provide advanced clinical assessment, treatment and support across COCO PCN.

The role combines home visits and practice-based clinics, assessing, diagnosing, treating and appropriately referring patients to provide safe, effective and timely care.

Working alongside GPs and the wider multidisciplinary team, you will support urgent and routine care, improve access to primary care, reduce avoidable hospital admissions and improve patient outcomes.

You will be expected to demonstrate strong clinical decision-making, work autonomously within your scope of practice, contribute to service development and deliver evidence-based, patient-centred care.

Key ResponsibilitiesPatient Care

• Provide assessment, diagnosis, treatment and follow-up for patients in their homes and GP practices.
• Take medical histories, perform examinations and arrange appropriate investigations and treatment.
• Develop and review care plans, escalating or referring patients where appropriate.
• Recognise and respond appropriately to deteriorating or acutely unwell patients.
• Provide health education and support patients and families with self-management and prevention.
• Work collaboratively with GPs, nurses, pharmacists, social care and the wider multidisciplinary team.
• Ensure clinical equipment is maintained and fit for use.

Person Specification
Essential

• HCPC-registered Paramedic.
• Advanced Life Support (ALS) qualification.
• Level 7 postgraduate qualification or equivalent advanced clinical experience.
• Strong clinical assessment, diagnostic and decision-making skills.
• Ability to work autonomously within scope of practice.
• Good understanding of UK healthcare regulations and professional standards.
• Strong communication, organisational and problem-solving skills.
• Compassionate, patient-centred approach with a commitment to quality care.
• Ability to remain calm and professional under pressure.
• Commitment to multidisciplinary and collaborative working.
• Full UK driving licence and access to a roadworthy vehicle.

Desirable

• Experience in primary care, community healthcare or home visiting.
• Experience of clinical audit and quality improvement.
• Working knowledge of EMIS.
• Additional community healthcare qualifications.
• Experience of working within or contributing to multidisciplinary teams.

Qualifications
Essential

• Essential Qualifications and Experience
• Registered Paramedic with the Health and Care Professions Council (HCPC).
• Educated to degree/diploma level in Paramedicine or an equivalent qualification.
• Holds a current Advanced Life Support (ALS) certificate.
• Has successfully completed the two-year Consolidation of Learning period as a newly qualified paramedic.
• Has a minimum of three years experience working at Band 6 or equivalent level as a paramedic.
• Holds, or is working towards, a postgraduate qualification in a relevant field, such as Advanced Paramedic Practice, Advanced Clinical Practice or Healthcare Management.
• Is working towards developing Masters-level or equivalent capability within their area of paramedic practice.
• Will be required, within six months of commencement of reimbursement (or a longer period where agreed with the commissioner), to have completed and been formally signed off against the clinical pillar competencies within the Paramedic FCP/AP Roadmap to Practice.

Desirable

• Working knowledge of computers and EMIS clinical system
• Additional qualifications in community healthcare
• Experience in a home visiting or primary/community healthcare setting.
• Qualified in Phlebotomy
• Postgraduate qualification in Advanced Paramedic Practice, Advanced Clinical Practice, Healthcare Management or another relevant discipline
