Job SummaryThe
Virtual Ward service is available 12 hours a day, 7 days a week.
The post holders will require assessment skills and have the ability to monitor and support patients via the Virtual Ward with complex needs through an acute episode in the community in order to avoid a hospital admission.
The post holders will work closely with the local community teams and ensure early escalation of unwell patients. They will therefore need to be motivated and innovative, with strong written and verbal communication skills, to support the development of the Virtual Ward.

Main duties of the job- Work with the care support team and other external providers to ensure the right care, right person, right time and right place
- Aim to enable patients to regain independence or achieve their rehabilitation potential, preventing unnecessary hospital admissions
- Work closely with the Senior Clinicians and junior staff, demonstrating effective communication, networking and liaison skills, ensuring quality outcomes
- Be responsible for the assessment and treatment of patients during a crisis, identifying complex and specialist needs and developing, implementing and evaluating programmes of care.
- Support and promote the on-going development of all care delivery within the patient's home/Care Home environment, enhancing the quality of care.
- Work with a high level of autonomy, within agreed guidelines, managing undifferentiated and undiagnosed problems by incorporating physical assessment and diagnosis via telehealth devices, supported by senior members of the MDT
- Determine the medical stability of patients referred and those currently on the caseload and promote the mental, physical and emotional health of a given population
- Assist the Senior Clinicians in identifying gaps in service and address these gaps ensuring the need of the patients are met
- Assist with audits and help identify areas for improvement
- Provide expert advice to patients, families/carers, health professionals and members of the multi-disciplinary team
- Clinically supervise junior members within the virtual ward

Person SpecificationEducation and QualificationEssential- Registered General Nurse/Paramedic with current NMC/HCPC registration
- Completion of clinical reasoning and physical assessment
Desirable- Assessor/mentorship qualification: ENB 998 or equivalent
Knowledge and UnderstandingEssential- Evidence of working at Band 5 level within the community or acute NHS setting.
- Knowledge of normal vital signs (NEWS2)
- Knowledge and experience of EPR systems
- Awareness of current healthcare issues, policies and implications for practice
- Broad range of complex medical and respiratory conditions
Desirable- Knowledge of working with Telehealth /Virtual Ward.
- Knowledge of /additional specialist area through in-house training & short course.
ExperienceEssential- Experience as a physiotherapist / Nurse in Acute Respiratory, Intensive Care, Community, Virtual Ward, Rapid Response, A&E
- Minimum 2years post qualification with experience of working within the acute NHS or community setting
Desirable- Leadership course and evidence changing practice.
- Experience of managing staff
Skills/AbilitiesEssential- Capable of working and making decisions under own initiative
- Evidence of innovative practice
- Able to present information, written and orally, in a clear and logical manner
- Excellent telephone and video call manner and ability to take clear accurate messages paying attention to detail.
- Ability to manage stressful situation
Desirable- Awareness of monitoring telehealth/ virtual ward
GeneralEssential- Basic IT skills
- Flexible to meet the needs of 12/7 day service
- Ability to work on-site at hub location.
