Job summary
Rapid Response/Urgent Crisis Response Clinician Band 6

(Newly qualified Band 5 Nurses will be considered for this role however must have previous community experience)

Work as part of a team of clinicians, therapist's rehabilitation assistants and Community Geriatricians, prioritising avoidance of unnecessary hospital admission. Assessment and treatment of patients during a crisis, identifying complex and specialist need and developing, implementing and evaluating programmes of care. In addition, facilitating early hospital discharge for patients back to their own home. To Support and promote the on-going development of all care delivery within the patients own home, enhancing the quality of care delivered. As a clinician working with a high level of autonomy, within agreed guidelines, managing undifferentiated and un-diagnosed problems using advanced clinical skills by incorporating physical assessment and diagnosis

Main duties of the job
Working as part of the Rapid Response team, the post-holder will provide assessment and treatment of patients, within their own home, during a crisis. The post holder will identify complex and specialist needs and will develop, implement and evaluate programmes of care.

Working as a practitioner with a high degree of autonomy, the post-holder will manage undifferentiated and undiagnosed problems using advanced clinical skills by incorporating physical assessment and diagnosis and be supported to do this by senior members of the MDT.

The post holder will also identify patients suitable for virtual ward monitoring and be proficient in fitting the appropriate technology following training and support from the team and will be responsible in assessing unwell patients at home on request of the virtual ward hub and be able to administer IV antibiotics and IV diuretics in the community.

  • A minor injuries unit in New Addington

EDUCATION AND QUALIFICATIONS
Essential
  • Registered General Nurse/Paramedic with current NMC/HCPC registration
  • Completing of clinical reasoning and physical assessment

Desirable
  • Willingness to undertake Independent ( V300) prescribing study.

KNOWLEDGE AND UNDERSTANDING
Essential
  • Awareness of current professional nursing and NHS issues including health promotion
  • Evidence of up to date knowledge and skills in long term conditions
  • Evidence of continuing professional development
  • Awareness of current healthcare issues, policies and implications for practice
  • Reflective approach to practice

Desirable
  • Knowledge of working with a Telehealth

Experience
Essential
  • Significant post qualification experience
  • Experience of working within the community setting

Desirable
  • Leadership course and evidence changing practice.
  • Experience of managing staff

SKILLS/ABILITIES
Essential
  • Evidence of advanced clinical skills i.e. Physical assessment, decision making and psychological therapies.
  • Communication skills
  • Evidence of innovative in practice
  • Capable of working and making decisions under own initiative
  • Administration of IV antibiotics and IV diuretics within the community setting

Desirable
  • Awareness of virtual ward monitoring

  • Car driver, current UK licence and access to car for work purposes.
  • IT skills
  • Flexible to meet the needs of 7 day, extended hours community service
