Job summaryWe are looking for a Clinical Coder / Document Administrator to join our Administration and Coding Team on a bank contract basis.
In this role, you will review, extract, and accurately record clinical data into our EMIS Web clinical system. Your work will directly impact patient safety, data quality, and the practices achievement of national healthcare targets such as the Quality and Outcomes Framework (QOF).
Key Responsibilities- Document Processing: Review and action incoming electronic correspondence (e.g. discharge summaries, clinic letters, A&E reports) via EMIS Web.
- Clinical Coding: Extract diagnoses, procedures, medication changes and test results, assigning accurate SNOMED CT codes to patient records.
- Workflow Management: File non-clinical information and escalate urgent actions or medication changes to the appropriate clinician.
- Medical Summarising: Summarise new patient records into EMIS when required.
- Data Quality: Identify missing or inconsistent information and liaise with clinicians to resolve issues.
- Compliance & Governance: Maintain confidentiality and comply with NHS Information Governance, GDPR and safeguarding requirements.

The primary purpose of the role is to accurately review, extract, code and record clinical information from incoming electronic correspondence within EMIS Web. The post holder will review discharge summaries, outpatient letters, A&E reports and other clinical correspondence, applying accurate SNOMED CT codes, maintaining data quality and ensuring urgent clinical information is escalated appropriately. The role supports patient safety, continuity of care and healthcare performance targets.
The post holder will be based either at Cumbria Health headquarters in Carlisle, or remote working may be available.

- Document Processing: Review and action incoming electronic correspondence including discharge summaries, clinic letters, A&E reports and other clinical communications via EMIS Web.
- Clinical Coding: Extract diagnoses, procedures, medication changes and test results and accurately assign appropriate SNOMED CT codes within patient records.
- Workflow Management: Identify actions required from incoming correspondence and route urgent items, medication changes or clinical tasks to the appropriate clinician/team.
- Data Quality: Identify inconsistencies, omissions or missing information within clinical correspondence and liaise with clinical staff to resolve issues.
- Information Governance: Maintain confidentiality and comply with NHS Information Governance, GDPR and safeguarding requirements.
- Code all incoming discharge letters and patient-related correspondence into the medical record and workflow to the appropriate clinician.
- Ensure problems are coded correctly so information is available to support continuity of care.
- Identify and task actions arising from incoming mail to the relevant person or practice team.
- Identify coding opportunities that support quality initiatives and accurate reporting.
- Undertake any other additional duties appropriate to the post as required.
Person SpecificationKnowledgeEssential- Excellent customer service skills
