Job summaryAn exciting opportunity has arisen for a part time Band 6 Community Practitioner posts (24hrs per week) to work in the Northumberland and North Tyneside Universal Crisis team -Older Persons Pathway. The team is currently based at St Georges Park Hospital, Morpeth.
We are looking to recruit a community practitioner who is passionate about the role, the successful candidate will be encouraged to be involved in service improvement and the delivery of care ensuring an enhanced and streamlined service. The successful candidate will be expected to work universally during times of high pressures within pathways as an ageless service.
The post holder will have clinical expertise in assessment, intervention, and evaluation in mental health care. The role will involve the Biopsychosocial assessment of older people who have presented in crisis. They will be required to provide specialist advice regarding mental illness to service users, carers and other practitioners.
Main duties of the jobThe successful candidates will have responsibility for the effective management of a shared caseload of older people referred into the service, The service provides the following functions: assessments, home based treatment, facilitating early discharge from hospital, and supporting the coordination of Section 136.
We work closely with other professionals such as CNTW Access Hub, MSOP, CTT, Care environments, Local Authority, Safeguarding and Police to ensure older people in are care are fully supported and safeguarded.
The service operates 365 days per year over a 24hr period, with shifts being allocated to meet service requirements. The applicant must be able to meet the needs of all of the core roles and shift requirement. The candidate must have a full driving licence, including access to a personal vehicle with business insurance, in order to cover the geographical area,.
The applicant should hold a professionally registered qualification and preferably have experience of working with older people, carers and their families. It is also essential that they will have experience of working within a mental health/ learning disability area.

Person SpecificationKnowledgeEssential- Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
- Mentorship/Practice Educator Training
- Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
- Proven track record of post registration clinical experience relevant to the post.
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
